FMA Website Updates and Version History

The database has been refactored to increase the display speed and create the ability to generate reports.

Generation 3 (Gen 3) of the Uniform Dataset (UDS) and the Family-Centered FMA was updated to the website. Links to PDFs of the new forms are below.

Users can now choose the Standard FMA or Family-Centered FMA when entering data.

The updates to the UDS (Initial Demographic Intake, Delivery, and Follow-Up forms) are listed below:

9. Height: listed now in inches only (not feet and inches)
13. Ethnicity: question added
19. Primary Diagnosis: diagnoses reorganized to be in alphabetical order. Autism, Developmental Delay, Mitochondrial Disorder, Seizure Disorder, and Spinal Muscular Atrophy (SMA) have been added.
20. Year of Onset: Unknown or Not Reported added.
24. Falls question reworded to state “How many times has the person accidentally or unintentionally fallen in the last 3 months?” accommodating for falls from children that may be purposeful.
27. Employment: answers adjusted to Employed/Student/Homemaker/Volunteer, Not Employed/Not a Student, Retired
28. Living Situation: Homeless added as an answer
29. Current Means of Transportation: Ambulance/Medical Transport added as an answer
33. Current Primary Device: Stroller added as an option
36. Current Equipment Manufacturer: Amy Systems, Etac/Snug Seat/R82/Convaid, Innovation in Motion, Merits/Avid Rehab, Motion Composites, Motion Concepts, and Panthera added
37. Current Device Accessories: Anterior Tilt and Dynamic Seating Components added

To speed up the system, the user dashboard no longer displays patient names for new form submissions.
